Certificate of Insurance Request
If you need a certificate of insurance for yourself or an entity
requesting a certificate, please complete the information below and

New Provider Applications
If you are a new provider requesting an application or an existing
MedPro provider adding a physician or mid-level provider to your
practice, you will want to work with one of experts to complete an
application online through the MedPro web site. Contact Eric

Continuing Medical Education
MedPro offers online Continuing Medical Education for its providers.
In turn, providers may receive additional discounts off of their
insurance premiums for completing these CME courses. Once
established with MedPro as an insured, you may access the MedPro
library of CME products at the MedPro website at
www.medpro.com/c/portal/layout?p_l_id=PUB.1001.1
and establishing your own user name and password.

How to Report a Claim

Please advise us that you have a new claim or suit to report
Our customer service representative will request some basic
information, and then explain how to introduce all pertinent
documents to our claims imaging center.
THE FOLLOWING PIECES OF INFORMATION WILL BE
REQUESTED WHEN REPORTING A CLAIM.
Phone and fax numbers where claims professional may reach you.
What documents, if any, did you receive?
If served with legal papers, when were the papers received?
What are the dates of treatment involved which form the basis of the
claim?
Our Call Center will be happy to immediately direct you to the claims
professional who will handle your matter. However, our experience
has been that your initial discussion with our claims professional is
greatly enhanced when they have had the benefit of reviewing any
pertinent documentation first. Once we receive the new claim, you
can expect, in most cases, to be contacted by our claims
professional within two to three days.
How to Report an Incident
(Circumstances where you believe that a claim could be made in the
future)

Advise us that you have an incident to report
Our customer representative will mail you an Incident Reporting
Form which needs to be completed in its entirety, with applicable
dates of treatment shown where indicated. An incomplete form may
not trigger coverage.
Once completed, you should fax the Incident Reporting Form to the
fax number provided at the top of the form, or mail the form to the
address shown on the form.
Once the forms are received, a confirmation letter will be sent
notifying you that we have received the matter and set it up as an
incident. A claims professional will contact you with any questions.
